Chest wall reconstruction is a surgical procedure aimed at restoring the integrity and function of chest wall, which may be damaged due to trauma, cancer, infection, or congenital defects. The plays vital role in protecting thoracic organs, supporting respiratory system, maintaining shape chest. Therefore, any defect deformity can have significant functional aesthetic consequences for patient. authors present case report Kenyatta National Hospital (KNH) dyspneic 47-year-old male patient with right anterolateral post-pneumonectomy previously complicated by bronchopleural fistula. Past attempts had utilized ipsilateral latissimus dorsi muscle, pectoralis major omental pedicled flaps limited success. A chimeric thigh (ALT) perforator-free flap vastus lateralis (VL) muscle was used obliterate intrathoracic dead space provide cutaneous paddle. This aims show versatility ALT prevent syndrome associated tracheal deviation, inspiratory stridor, exertional dyspnea. In conclusion, obliteration challenging needs careful planning execution.
